The RN Transplant will be responsible for coordinating a focused and effective effort between the transplant center and its major patient referral sources (e.g. Dialysis Units and Nephrology Clinics) for renal transplantation. Will assume a case manager role in assuring that potential transplant recipients and donors will be afforded timely; efficient transplant education, work up, updates, and activation for transplantation when directed. Will be responsible for a designated caseload of patients during the pre/post-recipient and pre/post-donor transplant process. Aspects will include coordination of referrals, responding to roadblocks within the transplant workup process, and providing patient, family, and staff educational programming. Works to assure policies, procedures, and all regulatory requirements are met to assure the transplant program stays in compliance with various governing agencies. May provide outreach services to regional dialysis and nephrology programs.
Works on multi-disciplinary teams involved in routine organ transplant procedures. Understands types of organ disease and the associated treatment and transplant methods. Offers effective feedback to patient and family members regarding health. Names and describes the regulatory agencies and organizations involved in the transplant process. Must possess a high level of critical thinking, reasoning skills, and positive, assertive communication skills. Effectively delegates and supervises based on one's state practice act and an understanding of the concepts of responsibility, authority, and accountability. Describes the major steps in the transplant process from initial evaluation to post-transplant care. Collects and maintains patient information used in internal & external transplant information systems. Assesses risks and benefits of organ transplants to help craft treatment plans for potential patients.
Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing required, or in the process of acquiring with an estimated completion date within five years.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Minnesota RN license is required within six months of date into position. Obtains and subsequently maintains required department specific competencies and certifications.
