The Licensed Practical Nurse (LPN) participates in the implementation and evaluation of patient care for stable and predictable patients of all ages in the home care setting. Demonstrates knowledge of nursing principles for the patient in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's status and to interpret appropriately the information needed to identify each patient's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients. Knowledge of the principles and skills needed for nursing to provide patient care and treatment. Knowledge of medications and their effects on patients. Knowledge of patient education principles to develop a self-care program. Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting. Skill in administering a variety of treatments and medications as directed. Skill in documenting and maintaining records. Skill in establishing and maintaining effective working relationships with patients, health care providers, and the public. Ability to recognize problems and recommend solutions. Ability to interpret, adapt, and follow guidelines and procedures. Ability to communicate clearly. Ability to maintain confidentiality of sensitive information. Reinforces healthcare teaching of patients and family members, under the direction of a registered nurse.
Adheres to legal regulations and organizational policies when discussing patient health information Applies home healthcare operating principles and practices within own position Communicate relevant information with patients and health team members to ensure quality and continuity of care. Contribute to the evaluation of the plan of care by gathering, observing, recording and communication patient responses to nursing interventions. Establishes and maintains productive working relationships within and outside of own area. Follows safety measures to ensure proper use of equipment and supplies. Implement interventions that are assigned, ordered or prescribed by licensed practitioner or registered nurse. Observe patients, document and report changes in patient condition, such as adverse reactions to medication or treatment, and take any necessary action. Participate and be part of a healthcare team to conduct focused nursing assessments and contribute to the plan of care. Participate in health teaching developed by a licensed practitioner or registered nurse. Promote a safe and therapeutic environment by providing appropriate monitoring and surveillance of the care environment. Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Valid drivers license and proof of insurance required. Basic Life Support (BLS) for health care providers required. Graduate from an accredited practical nursing program as a Licensed Practical Nurse. One year of professional nursing experience preferred.If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
